Is an oral appliance right for me?
Not sure if treatment is right for you? You are not alone.
Many people who come to SleepWise are not entirely sure whether their snoring or sleep apnoea warrants treatment - or whether an oral appliance is the right fit for their situation. This page is designed to help you think it through, honestly and without pressure.
There are no wrong answers here. Whether you have a formal diagnosis, a suspicion, or simply a partner who has had enough. This is a good place to start.
The honest answer
Oral appliance therapy is not right for everyone.
But for a great many people - particularly those who snore regularly, have been diagnosed with mild to moderate sleep apnoea, or have tried CPAP and cannot tolerate it - it can be genuinely life-changing.
You may be a good candidate if….
-
Snoring is the most common reason people come to SleepWise. Whether it is causing relationship tension, disrupting your partner's sleep or simply bothering you — an oral appliance is the frontline treatment recommended by sleep physicians and ENT surgeons for snoring.
-
Oral appliances are recommended by the Australasian Sleep Association and the American Academy of Sleep Medicine as the frontline treatment for mild to moderate OSA. If this is your diagnosis, an oral appliance is a clinically appropriate and well-supported first-line option.
-
Between 30 and 60% of patients prescribed CPAP are unable to use it long-term. If you have been prescribed CPAP and are not using it — for any reason — an oral appliance is a clinically proven alternative that many former CPAP users find far more comfortable and practical.
-
The Australasian Sleep Association and the American Academy of Sleep Medicine recommends that all patients be offered the choice between CPAP and an oral appliance — even for severe sleep apnoea. If CPAP is not working for you, an oral appliance is a legitimate clinical option worth discussing.
-
Many patients who use CPAP at home choose an oral appliance for travel or camping as it requires no power source, fits in a small case and does not require airline approval.
-
You do not need a diagnosis to make your first appointment at SleepWise. If you snore regularly or recognise symptoms of sleep apnoea, we can assess your situation, arrange a home sleep study if needed and guide you from there.
Oral appliance therapy is an effective alternative that is preferred by some patients. The American Academy of Sleep Medicine recommends that sleep physicians and dentists work together to identify the optimal treatment for each individual patient.

Is CPAP better than an oral appliance?
CPAP is the gold standard treatment for severe obstructive sleep apnoea and is the most efficacious therapy for reducing breathing disruptions during sleep. If you have severe sleep apnoea and are using CPAP successfully, it is likely the right treatment for you and we would encourage you to continue.
However, real-world outcomes tell a more complex story. A significant proportion of patients cannot tolerate CPAP long-term and an untreated patient carries the full burden of their condition regardless of the reason they are not using their prescribed therapy.
For these patients, oral appliance therapy is not a compromise. It is the treatment that actually gets used and studies consistently show it achieves comparable real-world outcomes to CPAP, driven by significantly higher compliance and patient tolerance.
95% of patients see improvement in snoring with a custom oral appliance
92% effectiveness demonstrated for sleep apnoea in clinical studies
Long term compliance rates with oral appliances of up to 90%, compared to 30 – 60% for CPAP.
Common concerns, answered honestly
-
Most patients adapt within the first two week and find the appliance surprisingly comfortable - often telling us they do not want to sleep without it. SleepWise uses only the latest generation 3D-printed nylon appliances, manufactured to as little as 2mm in thickness. They are custom-fitted to your teeth - not a generic device - which makes a significant difference to comfort.
-
A history of bruxism or TMJ sensitivity is not a reason to rule out oral appliance therapy. What it requires is careful assessment and attentive management - both of which are central to how SleepWise’s sleep medicine dentists work. Our clinicians conduct an assessment of your jaw and movement at your initial consultation and monitor your jaw health throughout treatment. Most patients with pre-existing TMJ or TMD concerns tolerate the appliance well and many find symptoms improve over time. Any jaw discomfort that is experienced is usually manageable with minor adjustments as guided by your clinicians.
-
The fee for a custom 3D-printed nylon oral appliance and treatment at SleepWise is comparable to the price of a CPAP machine on its own. Additionally, the fee includes all follow-up reviews and adjustments for the first 12 months and a three-year laboratory warranty with an 80% money back guarantee if you cannot adapt to your oral appliance despite our guidance. Most private health funds with extras cover provide a rebate. Payment plans are also available through Zip.
-
Most of our patients achieve excellent results - and if you are not among them, we will not leave you unassisted. Our clinicians will work with you to optimise your treatment and explore every modification. Where an underlying issue is contributing to your symptoms, we will arrange a referral to a sleep physician or ENT surgeon. If you are not able to adapt to your oral appliance despite the guidance of a SleepWise sleep medicine dentist, you are entitled to a refund of 80% of your treatment fee.
-
No, you do not need a referral to see a SleepWise sleep medicine dentist. You can book directly online or by calling our clinics - no GP or dentist letter required. If you have been referred by your GP, sleep physician or ENT, please bring any relevant documentation to your appointment.
-
A sleep study is highly recommended to undertake if you have any signs and symptoms of snoring or sleep apnoea. The information it provides will be important to establish a better understanding of your sleep. If you have already been diagnosed with sleep apnoea, bring your results to your first appointment. If you have not yet had a sleep study, we can arrange a home-based diagnostic sleep study from your initial consultation - conducted in your own bed, reported by a specialist sleep physician, with results typically available within 10 business days.
At SleepWise, more than twenty years of clinical experience has reinforced what the evidence consistently shows - a custom oral appliance is the best available treatment for snoring, and one of only two treatments recommended as frontline therapy for sleep apnoea.
Still not sure?
